WAIVER AND RELEASE OF LIABILITY
By checking box and printing my name, I acknowledge and agree to the following:
Assumption of RiskI am voluntarily participating in the Breath and Sound Festival on June 14th, which includes but is not limited to yoga classes, workshops, meditation sessions, physical activities, and other activities. I understand that these Activities involve physical exertion and carry inherent risks of injury or even death. I certify that I am in good physical health and have no medical condition that would prevent me from safely participating.
Release of LiabilityI hereby release, waive, discharge, and hold harmless Breath and Sound INC, its employees, volunteers, instructors, contractors, sponsors, and venue owners from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in the Activities, whether caused by the negligence of the Released Parties or otherwise.
Medical TreatmentI give permission for the Released Parties to arrange for emergency medical treatment, including transportation if necessary. I agree to be fully responsible for any costs related to medical care I may require.
Photography & Media ReleaseI understand that photos and videos may be taken during the event. I grant permission to use my likeness in promotional materials, social media, and publications without compensation or further approval.
COVID-19 & Communicable DiseaseI understand that attending the festival includes potential exposure to communicable diseases, including COVID-19. I accept all such risks and release the Released Parties from any liability related to such exposure.
Legal Capacity & Binding AgreementI certify that I am at least 18 years of age (or that a parent/guardian). This waiver is governed by the laws of Massachusetts and is intended to be a complete and unconditional release of liability.
